Position: Reablement Assistant

Location: 106 Townsend Ln, Liverpool, L6  0BB

Start Date: ASAP

Contract Duration: 13+ Month

Working Hours: 35 hours per week

Pay Rate: £ 112.00 per day

Job Responsibilities

Provide practical and emotional support to service users in line with agreed care and support plans

Promote independence by supporting individuals with daily living skills and reablement goals

Assist service users with personal care and use of appropriate moving and handling equipment where required

Support individuals with activities such as cooking, cleaning, laundry, and maintaining a safe home environment

Encourage and enable service users to build confidence and regain independence in daily routines

Work collaboratively with health and social care professionals to deliver coordinated, person-centred care

Participate in assessments, reviews, and updates of support plans

Support service users in social, recreational, educational, and therapeutic activities

Maintain accurate records and report any changes in service user needs or wellbeing to the line manager

Promote dignity, respect, independence, and equality in all aspects of care delivery

Follow all council policies, procedures, health and safety requirements, and safeguarding guidelines

Work flexibly across different service locations within the city as required

Attend supervision, training, and team meetings to support ongoing professional development

Person Specification

Must-Have Requirements

NVQ Level 3 in Health and Social Care, or willingness to work towards it

Experience working within an adult social care or similar support setting

Experience supporting individuals to maintain and develop independence

Ability to follow structured reablement or support programmes

Good communication skills (verbal and written)

Ability to work effectively with service users, families, and professionals

Strong listening and interpersonal skills

Willingness to work flexible shift patterns, including evenings and weekends

Commitment to training, supervision, and continuous professional development

Enhanced DBS clearance (Adult Workforce and Adult Barred List) required

Nice-to-Have Requirements

Experience delivering person-centred care approaches

Manual handling training or certification

Understanding of public sector values and commitment to equality and inclusion

Ability to work effectively as part of a multidisciplinary team
