Qualified Social Worker | Hospital Discharge Team | Brighton

• Qualified Senior Social Worker required to join a statutory Hospital Discharge Team
• Hybrid working
• Office base: Brighton
• You must have a social work qualification and full SWE registration as well as relevant experience to be considered
• Overseeing safe and timely discharge
• Supervisions
• Working closely with health partners, updating on calls, attending meetings
• Care Act oversights
• MCA's
• Pay rate £35-£37ph

To be considered, it is essential that you have a recognised Social Work qualification (Social Work Degree or Diploma in Social Work) and full Social Work England registration as well as recent or significant relevant experience in a similar role.
