Travel with Confidence Coordinator

Age UK Leeds is a leading local voluntary sector organisation providing high quality services to improve the health and wellbeing of older people in Leeds. 

Are you looking for a role that is varied and challenging, where you have the potential to make a real difference and improve the lives of older people in Leeds? If so, we at Age UK Leeds are recruiting for an Travel with Confidence Coordinator  to work as part of our Travel with Confidence Team.

Salary - Scale 6 £27,973 pro rata

Hours - 15 hours per week

Contract Term Service is contracted until 31st March 2027

Location – Leeds

 The Travel with Confidence Programme, along with other third sector organisations aims to support people aged 50+ to have more positive experiences of the transport system and feel more confident in making journeys.

This is a brand new project with the opportunity for the Co-ordinator to contribute to the planning, development and analysis of the service.

The focus of this role is to coordinate up to 12 weeks of person-centred intervention. You will lead on co-ordinating referrals, through the programme, assessing client’s needs and developing 121 outcome-based plans. 

Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers

You will work alongside communities, other voluntary organisations and a range of partner agencies to ensure that older people have the knowledge, resources and support systems in place to maximise resilience now and in the future.

The role will involve supporting paid Support Workers, carrying out case supervisions as well as holding a caseload of clients. The role also involves the coordinating and supervising volunteers.

Experience and knowledge of outcome-based support plans and producing risk assessments is a requirement for this role, along with experience of working with older people who have a disability.
