We are currently seeking a dedicated Social Worker with experience in community mental health care, ready for a new opportunity to make a difference in Slough.

A Day in the Life of a Community Social Worker

As a social worker specialising in community mental health care, your role is pivotal in providing support and care to patients:

• Conducting home visits to assess and ensure the well-being of individuals within the community.
• Administering medications according to prescribed schedules to address patients' needs.
• Engaging in empathetic conversations with patients to comprehend their conditions, facilitating effective assistance and monitoring their progress or setbacks.
• Performing risk assessments when necessary to ensure patient safety and promoting a secure environment during treatment.

Requirements and Qualifications:

• Active registration with the National Association of Social Workers or equivalent accreditation.
• Demonstrated experience in community mental health care.
• Strong communication skills and ability to thrive in high-pressure environments.
• Ability to work autonomously and collaboratively within a multidisciplinary team.
• Commitment to delivering compassionate and high-quality care to patients.
