Are you a dedicated and compassionate Practice Nurse seeking a fulfilling role within a forward-thinking, innovative, and progressive Practice?

We have partnered with a family-friendly practice located in West Yorkshire who are excited to welcome a new Practice Nurse. They are a CQC 'Good' rated practice, serving 13,000 patients with a branch surgery close by. They emphasise collaborative working and prioritise health & well-being. With a broad mix of patients across all age ranges, they focus on managing long-term conditions.

Position Details:

  • Job Title: Practice Nurse.
  • Job Type: Full time or part-time flexible hours available.
  • Location: Wakefield region.
  • Salary: £40,000 - £46,000 per annum with an extensive benefits package including personal development opportunities (CPD leave entitlement) plus NHS pension, 6 weeks annual leave + bank holidays, on-site parking and much more.
  • Team: Led by 4 GP Partners and a dedicated Practice Manager, their clinical team includes a salaried GP, 3 Advanced Clinical Practitioners, an ANP, and a Practice Nurse with Asthma/COPD training, plus dedicated support staff including receptionists, an administrative team, and prescription administrators.

About You:

  • NMC Registration with experience working in primary care.
  • General Practice Nurse experience in chronic disease management with a diploma in diabetes being highly advantageous.
  • Immunisations experience as well as wound care, cytology etc.
  • Excellent communication skills, effective time management, ability to work autonomously and as a team member.
  • Polite, confident, motivated, forward thinker, problem solver, high levels of integrity and loyalty, sensitive and empathetic.

Main Duties:

  • Responsible for providing comprehensive nursing care to patients, including chronic disease management, ensuring patient care plans are updated and regularly reviewed.
  • Routine immunisations/ childhood immunisations.
  • Wound care and treatment of minor injuries.
  • DES/QOF related clinical and administrative work as well as produce accurate, contemporaneous and complete records of patients consultation.
